Agency Application

Here is a review. We decided where we wanted to adopt from and then we interviewed and chose and agency. The next step in our adoption process was submitting our agency application. This application was only for the agency we selected (not for the country we selected, China). The agency basically used this information to see if we qualified to adopt from China before going through the printer breaking and expensive work of compiling our dossier.

The information our agency wanted in the application was:

• Standard Information (name, address, citizenship, birthdate, etc)
• Marriage Information (anniversary, previous marriages)
• Who is living in your household
• Arrest History
• Health Information (BMI, height, weight, chronic illnesses)
• Insurance Information
• List of extended family
• References
• Financial Information (employer information, assets, liabilities)

The application took about 30 minutes to fill out. We mailed it to our agency along with a $150 review and assessment fee.

The information our agency requested in the application is basically the same information we had to gather for our dossier. The difference is, you must verify your answers for your dossier (i.e. instead of filling your marriage info in a blank, you must provide a certified copy of your marriage certificate).

The agency application was the easiest step in the paperwork. One thing to note is once our agency approved our application (around a week later), we had to pay another fee ($1090) in order to start our dossier paperwork. Just be aware, because that payment can sneak up on you.
